{"id":16336,"date":"2023-04-14T13:15:04","date_gmt":"2023-04-14T11:15:04","guid":{"rendered":"https:\/\/mbits.info\/?p=16336"},"modified":"2023-06-14T11:39:16","modified_gmt":"2023-06-14T09:39:16","slug":"mbits-im-clinical-neuroradiology","status":"publish","type":"post","link":"https:\/\/mbits.info\/en\/mbits-im-clinical-neuroradiology","title":{"rendered":"mbits in Clinical Neuroradiology"},"content":{"rendered":"\n<p>In der neuesten Ausgabe des Journals \u201e<a href=\"https:\/\/link.springer.com\/article\/10.1007\/s00062-023-01276-0\" target=\"_blank\" rel=\"noreferrer noopener\">Clinical Neuroradiology<\/a>\u201c wurden die Forschungsergebnisse des mbits-Mitarbeiters Gregor Nageler ver\u00f6ffentlicht, der sich im Rahmen seiner Doktorarbeit bei uns mit der automatisierten Vorhersage des Erfolgs einer Thrombektomie besch\u00e4ftigte. Unter der Betreuung unseres CEO Ingmar Gergel und CTO Markus Fangerau entstand in Zusammenarbeit mit der Neuroradiologie der Uniklinik Heidelberg das <a href=\"https:\/\/mbits.info\/3d-flip-book\/mbits-clinical-neuroradiology\">Paper<\/a>: \u201eDeep Learning-based Assessment of Internal Carotid Artery Anatomy to Predict Difficult Intracranial Access in Endovascular Recanalization of Acute Ischemic Stroke\u201c.<\/p>\n\n\n\n<p>Ein wichtiger Pr\u00e4diktor f\u00fcr den neurologischen Outcome bei einem Schlaganfall ist die zeitliche Dauer der Thrombektomie. Ziel der Studie war es, anhand einer Deep Learning basierten Analyse der computertomographischen Angiographie (CTA) automatisch komplexe F\u00e4lle zu erkennen.<\/p>\n\n\n\n<p>Zun\u00e4chst wurde daf\u00fcr ein neuronales Netzwerk trainiert, welches automatisch den Aortenbogen und die Halsschlagadern in CTAs segmentiert. F\u00fcr das Training des Algorithmus wurden 121 Datens\u00e4tze aufw\u00e4ndig manuell segmentiert. Der Dice Score, welcher das Ma\u00df an \u00dcberlappung zwischen den manuellen Segmentierungen und denen des Netzwerks angibt, lag bei 0.94. Anschlie\u00dfend konnte das Netzwerk verwendet werden, um 258 weitere CTA Aufnahmen automatisch zu segmentieren.<\/p>\n\n\n\n<p>Die nun insgesamt 379 Gef\u00e4\u00dfsegmentierungen wurden in zwei Klassen eingeteilt, abh\u00e4ngig davon, ob in der Arteria Carotis Interna (ICA) ein spitzer Winkel (&lt;90\u00b0) oder nur stumpfer Winkel (>90\u00b0) vorkommen. Diese Einteilung basiert auf einer k\u00fcrzlich erschienenen Ver\u00f6ffentlichung [1], welche den Zusammenhang zwischen spitzen Winkeln in der Halsschlagader und einer l\u00e4ngeren Dauer der Thrombektomie zeigt. Dieser Zusammenhang konnte auch in dem Datensatz dieser Studie durch einen Whitney-U-Test reproduziert werden. W\u00e4hrend die Eingriffszeit in der >90\u00b0 Gruppe bei 48 min betrug, lag sie bei der &lt;90\u00b0 Gruppe bereits bei 64 min (p=0.001).<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img fetchpriority=\"high\" decoding=\"async\" width=\"360\" height=\"295\" src=\"https:\/\/mbits.info\/wp-content\/uploads\/2023\/04\/Fig2measurement_angles.jpg\" alt=\"\" class=\"wp-image-16344\"\/><\/figure>\n\n\n\n<p>Der bin\u00e4re Datensatz wurde verwendet, um ein zweites Netzwerk zu entwickeln, welches anhand der Segmentierung erkennt, ob in dem Gef\u00e4\u00df ein spitzer Winkel vorkommt oder nicht. W\u00e4hrend 90\u00b0 Winkel im zweidimensionalen auf einen Blick zu erkennen sind, ist die Suche nach einem solchen Winkel in einem dreidimensionalen Gef\u00e4\u00dfbaum zeitintensiv, weshalb eine automatische Klassifizierung an dieser Stelle vorteilhaft. Das Netzwerk konnte die spitzen Winkel mit einer Genauigkeit von 85 % erkennen.<\/p>\n\n\n\n<p>Die in der Studie entwickelte Deep-Learning Methode kann basierend auf der CTA Aufnahme automatisch komplexe Thrombektomief\u00e4lle erkennen.
